11. The physician orders an initial dose of digoxin for a 1 year old child weighing 22 lb. 

Order: digoxin (Lanoxin) 0.25 mg IV stat. 

The drug book recommends the following for a child <2 years old: digoxin 30-50 mcg/kg in 3 or more divided doses with the first dose equaling about ½ of the total.
